INSURANCE CLAIM INFORMATION

1. Copy of the Rotary Youth Exchange Policy Number #GLB 9024420 from the website www.culturalinsurance.com
   Click on Rotary International at the bottom on the left side
   Click on CIS-Bolduc Rotary Youth Exchange 
   Click on How to file a claim and there is the policy #, address and phone #
   
   2.  Copy of the Insurance claim form to the host family, since they will most likely be the first contact with health care providers
   
   3.  Copy of the Medical Release form to the host family.  YEO may be out of reach when it is needed.  The Club President can always step in.
   
   4.  Photo Copy of Passport/Visa and J1 form.
